Staff Can Earn Points Toward Rewards!

Do you, a patient or family member want to recognize a team member’s great work? Submit a Star/CareGram!

While longtime staff at SMH-Sarasota may remember StarGrams as a paper-based employee reward & recognition program, Sarasota Memorial has transitioned the program to an online version. The paperless program first launched at SMH-Venice in 2021 and is now available at the SMH-Sarasota campus. Future phases of the rollout will add off-site locations.

The program allows patients, family and team members to recognize an employee’s great work by filling out an electronic Star/CareGram form. Staff members earn three points for each Star/CareGram they receive and can redeem their accumulated points for a variety of rewards.

Here’s how the program works at both hospitals:

How to Send a Star/CareGram:

• Star/CareGram business cards with a QR code are now available in patient care areas and in patient admission folders at our two hospital campuses. Note: There are separate QR codes and forms for the SMH-Sarasota and SMH-Venice campuses.

• Patients, family and co-workers should scan the QR code with their smartphones to access the online Star/CareGram form.

• Please include the team member’s first and last name when filling out the form.

• The completed submission form will automatically be sent to managers for their review/approval.

• Nursing leaders submit a CareGram using an iPad software tool during rounding in patient care areas — this has NOT changed.

How to Redeem a Star/CareGram for Rewards:

• Employees will receive a notification via their SMH email each time they receive a StarGram or CareGram. The email notification will include the employee’s total number of points earned to date, and a link to an online form to redeem points for rewards.

• Rewards include a range of SMH-branded apparel and gifts, as well as vouchers to cafés and Gift Shops at each hospital campus.

• Employees will be notified via email when their reward is ready to be picked up at their hospital campus Gift Shop.
