Benefits Reminder: Earn Incentive for 3 Wellness Activities
Make a resolution in the new year to earn an incentive for taking steps towards better health.
The Healthy Merits Wellness Program, open to all employees enrolled in SMH health plans, will continue to offer a special $15 per pay period premium credit as of October 2024 to those who complete wellness activities in the new year.
A similar program was offered this past year; to maintain the wellness incentive effective in October 2024, employees will need to complete all three of the following activities by Sept. 30, 2024, including a preventive care exam with their primary care provider.
1. NicotineFree: Self-attest to being nicotine free at https://smh.healthymerits.com. If you are a tobacco user, you can complete the tobacco cessation online courses for a reasonable alternative standard.
2. AgeGage: Complete a 15-minute online health assessment found at https://smh.healthymerits.com

3. PreventiveCare: Complete a preventive exam with your primary care provider between Oct. 1, 2023 and Sept. 30, 2024 and self-attest the date and type of exam at https://smh.healthymerits.com.
